Adding Custom Fields to the Registration Form

You can add up to four custom fields on your registration form. This gives you the option to capture additional information from your Learners at sign up if required.

Important: If your platform is a Dental GDC accredited platform then Job Title and GDC Number are already included as optional fields on your registration form.


In this article we cover how you add a custom field to your registration form, what controls you have over your custom fields, and where you can find the information provided by your Learners.


How do I add a custom field to the registration form?

You can add up to four custom fields to your registration from the 'Platform Set-Up' page which is found in 'Settings'.

To add a custom field to your registration form:

  1. Click 'Add'
  2. Enter the name of the custom field
  3. If you want to make it a mandatory field for Learners to complete at registration then you also need to check the box to confirm this
  4. Then click 'Add'

  • If the input field is mandatory, the registration form cannot be submitted until the Learner has entered some text in the new field.
  • If it is not mandatory then the Learner can complete your registration form without providing the information.

Once you have added your custom field it will be enabled by default and will immediately be displayed on your registration form as shown in the images below.


Can I control the order in which the custom fields will appear on the registration form?

The custom fields will always appear after the existing registration input fields.

However you do have control over the order in which your custom fields are displayed on your registration form.

You can drag and drop the order they should appear in. Just click on the icon in the 'Order' column as highlighted by the green box in the image below and you will be able to drag the custom field into the correct position.

In this example we've moved 'Practice Name' to appear after 'Contact Number'.

'Contact Number' will now appear above 'Practice Name' on the registration form as shown in the images below.

Where can I find the information provided by Learners in these custom fields at registration?

The information entered in your custom fields by Learners at the time of registration can be found in your Learners report which is accessed via the 'Learners' page in the Administrator Dashboard.

  1. Go to 'Learners'
  2. Click 'Export All'
  3. A .csv file will be downloaded which includes the information for your custom registration fields

Can I edit my custom fields?

Yes you can edit the field at any time by clicking on the pencil icon.

Important: If you change the name of the custom field, any information provided previously by Learners will now appear against the new custom field name in your Learners report.

For example if your field was called 'Practice Name' and Learner One provided the input 'Practice One', then if you change the name of the custom field to be 'Contact Number', Learner One's information (Practice One) will appear under the column 'Contact Number' in your .csv report.


How do I remove a custom field from my registration form?

You can disable a custom field by toggling the field red as shown below. It will then no longer be displayed on your registration form. All information provided by your Learners will still be stored and displayed in your Learners report.

Alternatively, once disabled you are also able to delete your custom field altogether by clicking on the delete icon.

Important: If you delete a custom field after it has been used by a Learner, this information will be deleted and will no longer appear in your Learners report. Therefore please export the data you need before deleting the custom field.


Can the Administrator or my Learners edit the information provided in the custom fields after registering?

No not currently. We are exploring adding this functionality in later releases.


Can the Learner view the information provided in the Custom fields on their profile page?
