How to create a questionnaire/test
This article covers how you add an assessment or questionnaire to a course.
You can add one questionnaire/test per module and choose from four question types which we cover below. These question types are; 1) Multiple Choice, 2) Yes/No, 3) Sequence, 4) Matching Pairs.
How do I add a questionnaire/test to a course
You can add one questionnaire/test per module. It can be the only item in the module or in combination with Learning Resources/Webinars/Face to Face Lessons/Assignments. And you can set a pass mark if you want Learners to pass the test before being able to complete the course.
Learners will be able to access the questionnaire/test once they have completed all learning resources/Webinars/Face to Face Lessons or Assignments in the module (if there are not any they can access it straight away) and they must complete (and pass) the questionnaire/test in order to complete the module.
Below are the steps to adding a questionnaire/test to your module:
1. Create a new module and save a module title - the module content fields will appear allowing you to add Learning Content (if you wish) and add a module questionnaire/test.
2. Scroll down to the 'Questionnaire' section and toggle the feature on (so that it changes from red to green) - the 'Manage Options' section will appear
3. Click 'Add question' to start adding your test questions and answers - a modal will open allowing you to add your first question. You can add as many questions as you want.
Make sure you enter and save your question and answers (you can enter up to 255 characters for each question and answer) - you have four options when it comes to the type of questions you can ask:
- Multiple Choice: You can enter as many answers as you like and set which answers are correct and which answers are wrong using the toggle that appears alongside each answer. More than one answer can be correct.
- Yes/No: This is true/false question - you can only enter two answers, one will be 'Yes' and one will be 'No'. You need to toggle which answer is correct and which is incorrect. Only one answer can be correct.
- Sequence: If your question requires Learners to place answers into the correct order choose this option. You'll need to enter your answers and set what the correct order is (you can drag and drop your answers into the correct order). You'll see the numbers 1,2,3 etc on the left hand side of each answer which signifies the correct order.
- Matching Pairs: This question type allows you to present a lead-in statement/question with two lists of 'prompts' and 'answers' to the learner. The learner will be presented with the answers in a randomised order, and asked to drag and drop them to match the correct prompt. e.g. From the list below match the capital city to the correct country, or match the appropriate treatment to the symptoms described.
4. Make sure you click 'Save' and you'll be returned to the questionnaire overview section from where you can add another question and review the questions you have already added by expanding each of the questions to double check the answers to each question are all correct - click the '>' icon to expand the question.
- Green signifies a correct answer
- Red signifies an incorrect answer
5. Once you have added all your questions and answers, make sure you've set your pass mark. Click 'Save' at the bottom of the Module Overview page to save the pass mark.
0% means there is no required pass mark. 100% means Learners have to get all answers correct in order to pass the test and complete the module. But you can set your desired percentage pass mark (e.g. 50%).
Troubleshooting
Why is the module containing my Questionnaire marked as invalid? It's preventing me from publishing my course.
You need to ensure that:
- You have at least two answers for each question.
- That at least one of your answers is marked as correct (toggle is green).
You can check these rules have been met from the 'Manage options' section on the module overview page.
In the following example you can see that the last question contains no answers. To solve this click the edit icon and add the answers to the question - you need at least two possible answers and at least one of these needs to be correct.
You can edit your questions and/or answers by clicking on the edit icon next to the question. This will open up a modal from which you can edit your questions and answers. You can also change which answers are correct and incorrect.
Note: You cannot edit the question 'Type'. For example if it's a multiple choice question and you want to change it to a 'Sequence' you cannot do this. You would have to delete the question and add a new one instead.
For example, in the following question the first answer needs to be changed from correct to incorrect. By clicking the edit icon next to the question this change can be made.
